PicoSure vs Fraxel: Which Laser Suits Which Problem

These two get compared constantly, usually as if one is a newer version of the other. They are not. They do different jobs, and the decision is far less about which is superior than about what is actually wrong with your skin.
Two different mechanisms
PicoSure works on time rather than heat. Its pulses are so short that pigment is broken apart by a photomechanical effect before much heat spreads into surrounding tissue. That is why it can address pigment with relatively less thermal impact on the skin around it.
Fraxel does something fundamentally different. It creates thousands of microscopic treatment columns, leaving untreated skin in between to speed healing. The skin's repair response is the point — that is what improves texture, fine lines and scarring over the following months.
Matching the laser to the actual complaint

That is pigment. PicoSure is the more direct conversation.

That is texture. Resurfacing is what rebuilds it.

Common. We treat them in sequence, not simultaneously, and we see how your skin responds before committing to a course.

That may be melasma, and the plan changes substantially. Aggressive settings are the wrong instinct here.

What downtime really means for your trip
This matters more than people expect, particularly for visitors. Patients often plan a resurfacing treatment for the start of a holiday and then spend days avoiding photographs. I would rather tell you that in advance than have you discover it.
- Resurfacing leaves visible redness and a rough texture for several days. Makeup over it is not comfortable immediately.
- Sun exposure afterwards is the fastest way to undo the result and risk darkening.
- Saunas, jjimjilbang and hot yoga are out while the skin settles.
- If your trip is short and photographs matter, a pigment-focused session is usually the more sensible choice.
- Flying itself is fine; the dry cabin air simply means more moisturiser.

What I will not do
I will not sell a package of both before seeing how your skin handles one. I will not run resurfacing on skin that was in strong sun last week. And if your concern is pigment that behaves like melasma, I will say so even when you arrived wanting the stronger treatment — because the stronger treatment is how melasma gets worse.

What's the core difference between PicoSure and Fraxel?
PicoSure delivers energy in extremely short pulses that shatter pigment mechanically rather than by heating it. Fraxel creates controlled microscopic columns of injury that prompt the skin to rebuild, which is what improves texture and scarring. Pigment problem versus texture problem is the shortest way to think about it.
Which has more downtime?
Fraxel generally has more. Expect redness and a sandpapery texture for several days depending on the settings used. PicoSure is typically milder, with redness settling faster, though this varies with how aggressively it is run.
Which is safer for melasma?
Neither is automatically safe for melasma, because melasma reacts badly to heat and inflammation regardless of the device. Low-energy approaches with careful spacing are the general principle, and the decision needs an examination. Melasma treated aggressively with either laser can rebound darker.
Can I have both?
Sometimes, on separate visits, when someone has both pigment and textural concerns. They are not combined in one session, and I would not recommend buying a package of both before we have seen how your skin responds to one.
How many sessions before I see something?
Pigment often shows visible change sooner because you are seeing pigment clear. Texture and scarring build gradually over months as collagen remodels, so judging a resurfacing course after one session is judging it too early.
