Freckles vs Melasma: How to Tell, and Why It Changes Everything

This is the distinction I most wish people knew before they book anything. In the mirror both look like 'brown marks on my cheeks'. Under a lamp they are different conditions living at different depths, driven by different things, and the treatment that clears one can inflame the other.

I see the consequence of getting it wrong regularly — patients who had an aggressive pigment laser somewhere, saw a lovely result for three weeks, and then watched the patch return darker than it started. That is not bad luck. That is melasma being treated as if it were freckles.

What each one actually is

The heat sensitivity of melasma is the part people find surprising. It is not only ultraviolet light — a hot kitchen, a sauna, or a long summer commute can be enough to darken it. This is also why some patients find their melasma worsens after any treatment that warms the skin, regardless of what that treatment was aimed at.

Why the wrong diagnosis is expensive

   Strong pigment-targeting energy breaks up melanin visibly. Three weeks in, the skin looks clear and the patient is delighted.

   Under the surface, heat has provoked the pigment-producing cells that drive melasma.

   Usually between one and three months. Now the patient has spent money and has a worse starting point.

   Recovery is slow

   Settling a rebound takes months of gentle management, sun discipline and patience — considerably longer than the original treatment course would have taken.

How each is approached

For freckles, the plan is usually straightforward: target the pigment, then protect the result. The treatment itself is the smaller half of the job — without daily sun protection they return, and no clinic can prevent that for you.

For melasma, the approach is deliberately gentler and slower. Lower energy, more sessions, a longer view, and a real focus on the things that trigger it. I would rather take six months to get you somewhere stable than four weeks to get you somewhere that collapses.

• Sunscreen daily, reapplied — this is the treatment, not the aftercare.
• Avoid heat on the face where you can: saunas, hot yoga, prolonged cooking over steam.
• Tell me about hormonal contraception or pregnancy. It changes what is realistic.
• Expect maintenance. Melasma that is controlled still needs attention.
• Do not chase a fast result. The fast result is what causes the rebound.

When both are present

This is common and it is the situation that most needs an experienced eye. You may have freckles you have had since childhood sitting on top of a melasma patch that arrived in your thirties. Treating the whole area at freckle settings darkens the melasma underneath. The plan has to address them separately, and that means the examination has to distinguish them properly in the first place.
