Itchy Mole: When a Doctor Should Look Before Anyone Removes It

Every few weeks a visitor shows me a mole and says, almost apologetically, that it has been itching, and could I just take it off while they are in Seoul. An itchy mole is a fair thing to worry about, and it is exactly the kind of mole I slow down for. This page explains what I check first, why itch alone is often harmless, and when removal becomes a reasonable choice.
Why an itchy mole jumps the queue in my consultation room
Most moles patients bring me are about appearance: a dot on the cheek they dislike, a raised spot that catches a razor. An itchy mole is different, because itch is one of the ways skin tells you something has changed. Usually the change is trivial. Sometimes it is not, and the only person who can sort the two is a doctor looking at it in good light.

Itch alone, or itch plus change? A two-minute mirror check
Before you message any clinic, take a clear photo of the mole next to a ruler and answer the questions below honestly. The ABCDE letters are the general guide many doctors give the public. They help you decide how soon to be seen. They do not tell you what the mole is.
Harmless reasons a mole starts itching
When an itchy mole turns out to be ordinary, the cause is usually something in the skin around it rather than the mole itself. These are the stories I hear most often, and they tend to share one feature: the itch has a trigger and settles when the trigger stops.
- Dry skin in winter or after long flights, which makes the whole area itch, the mole included.
- Friction from a bra strap, waistband, collar or bag strap rubbing a raised mole.
- Shaving over a raised mole, leaving a small nick that itches as it heals.
- Sunburn or a new skincare product irritating the skin around the spot.
- Eczema or a rash that happens to include the area where the mole sits.
If you can name a trigger like these and the mole looks exactly as it did last year, the itch is very likely irritation. Remove the trigger, moisturise, and give it two weeks. If it is still itching with no reason you can point to, have it looked at.
What I actually do when you show me an itchy mole

I ask about the timeline
How long it has itched, whether it has changed, whether it has bled, and whether anything was rubbing or irritating it.

I examine it under good light and magnification
I look at the colour pattern, the edge, the surface and how it compares with your other moles.

I compare it with your own photos
Old photos on your phone are surprisingly useful. A mole that has not changed in five years tells a different story from one that is new.

I tell you which path it belongs on
Either it looks like an ordinary cosmetic mole that can be removed, or it needs a medical evaluation first, and I say which in plain words.

Cases where I refer you on instead of treating
Laser removal vaporises the pigment, which means there is nothing left to send for examination. That is perfectly fine for an ordinary mole and exactly wrong for one that needs a closer look. So there are moles I decline to laser, even when the patient has flown in and asks nicely.
- A mole with several of the ABCDE features, or one you tell me has clearly changed.
- A mole that has bled, crusted or ulcerated without an obvious injury.
- A new dark spot in an adult that looks unlike your other moles.
- Any spot where my examination leaves me unsure.
In those cases I explain why and suggest you have it evaluated by a dermatology department that can examine the tissue, either in Korea or at home. You do not pay for a removal I would not stand behind.
Once it has been cleared: removing an itchy mole for comfort
Many itchy moles are simply raised moles in an unlucky place, catching on clothing or a razor every day. Once I am comfortable that a mole looks ordinary, removing it for comfort is a reasonable choice. Should I worry about an itchy mole?
Itch on its own is often caused by something harmless, such as dry skin, friction from clothing or shaving. It becomes a reason to see a doctor promptly when the itch comes with a change in size, shape, colour, bleeding or crusting, or when it keeps returning without an obvious cause.
Can I get an itchy mole removed with a laser in Korea?
Only after a doctor has examined it and is comfortable that it looks like an ordinary cosmetic mole. If anything about it needs a closer look, a laser is the wrong first step, because laser treatment does not leave tissue that can be examined afterwards.
What does the ABCDE check mean for moles?
It is a general guide people use at home: Asymmetry, irregular Border, uneven Colour, a Diameter larger than a pencil eraser, and Evolving, meaning any change over time. It helps you decide when to see a doctor. It is not a way to diagnose yourself.
Why does my mole itch after shaving or a sunburn?
Shaving can nick or irritate a raised mole, and sunburn dries and inflames the skin around it. That kind of itch usually settles as the skin heals. If it does not settle within a couple of weeks, or the mole itself looks different, have it checked.
Do I need an appointment just to have a mole looked at in Gangnam?
