Neck Thread Lift: Which Necks It Suits, Where It Falls Short, and What I Suggest Instead

The neck question usually arrives as a gesture rather than a sentence: a patient pinches the skin under their chin and asks whether threads can fix it. I hear some version of "can I have a neck thread lift?" regularly, often from people who were happy with their face treatment and now notice their neck looks older. My honest answer depends on what exactly is making the neck look that way, so this page is about telling those causes apart.

Before you message any clinic about a neck thread lift, stand in front of a mirror and try this. It will not diagnose anything, but it will help you describe your neck accurately, which makes any consultation far more useful.

Is the line from your jaw to your neck still clear, or has it softened into a slope?

Does loose skin fold under the chin, and how much?

Do vertical cords appear in the front of the neck? Those are muscle bands, not loose skin.

Most patients have a mix. The job of the consultation is to work out which one dominates, because a neck thread lift addresses the first row well and the others poorly.
When a neck thread lift makes sense
The neck I find suits threads well has early softening right under the jaw, skin that still has some substance when pinched, and an owner who wants a subtle, tidier line rather than a dramatic change. In that neck, threads can support the tissue and stimulate collagen as they dissolve, and the jawline often matters as much as the neck itself.

The difference in the neck is that the skin moves constantly, so I am more conservative with how many threads I place and how much lift I ask for.
The honest limits of threads below the jaw
- The neck bends, turns and stretches all day, which places more strain on threads than a cheek does.
- Neck skin is often thinner than facial skin, so threads placed too close to the surface are more likely to be felt or cause puckering.
- Threads do not change the texture of crepey skin or erase horizontal creases.
- They cannot remove a large amount of loose skin; only surgery repositions and removes it.

Neck alternatives I often suggest instead
When laxity is spread across the lower face and neck rather than concentrated in one line, an energy-based lifting device is often my first suggestion. When the main complaint is thin, crepey neck skin, a collagen-stimulating injectable such as Radiesse is often a better fit than threads, because it targets skin quality rather than lift. Sometimes the honest answer is a combination over time: a device for overall tightening, then threads for a specific line if one remains. I would rather plan that sequence with you than put everything into one session.
Necks I would not treat with threads
I will not recommend a neck thread lift for heavy, hanging neck skin, because the result would disappoint you and a surgical opinion is the right next step. I also would not place threads in a neck with active skin infection or inflammation in the area, or while a previous thread problem is still unresolved.
If the main issue is muscle bands that appear when you clench, threads are the wrong tool. And if you need to be on a long-haul flight or turning your head constantly for work in the first days, I may suggest waiting for a better time rather than rushing.

It can help when the problem is mild to moderate laxity along the jaw-to-neck line in skin that still has some thickness. It does much less for neck bands, deep horizontal lines, crepey skin texture or heavy loose skin, which usually need a different approach.

Usually someone with early softening under the jaw, reasonable skin thickness and realistic expectations of a subtle change. A doctor should examine the neck in different positions before recommending threads.
