Thermage Side Effects Start Before the Treatment: A Risk Check I Run With Every Patient

Questions about Thermage side effects reach me every week, usually from patients who have read a frightening post and want to know if it could happen to them. My honest answer is that many outcomes people fear are decided before the tip ever touches the skin: by who is treated, where, and how. So this page starts before the treatment, with the check I run with every patient.
Thermage side effects start with your own history

Thermage delivers RF heat into the dermis and the layer beneath, contracting collagen and prompting new collagen over the following months. The same heat that firms the skin is what causes trouble when it lands on the wrong face, or in the wrong amount.
A pre-booking risk self-check
Answer these honestly before you book, and bring the answers to your consultation. A "yes" does not rule Thermage out; it tells me what to change or check first.
Face and eyes: the side effects differ by area
On the face, the tenderness patients notice most is along the jawline and over bony areas, and redness usually fades within hours. Swelling, when it happens, tends to be mild and short-lived.
Around the eyes, the skin is much thinner, so temporary puffiness is more visible, especially the next morning. Eye Thermage uses its own tip and a protective eye shield, and I plan it as a separate treatment with its own shot count, never as an extension of the face.
Neither area should blister, weep or develop a dent. Those are the signs I want to see a photo of the same day, wherever you are.
What I change when your risk is higher
- Energy level: lower over thin skin and bony areas, adjusted to your feedback as we go.
- Shot placement: fewer passes over hollow areas, more where the skin is thicker and looser.
- Timing: postponing after a tan or recent laser rather than treating irritated skin.
- Plan: sometimes a different treatment altogether, or volume or skin quality first.

When I would rather you skip Thermage
- You have very little facial fat and already feel your face looks hollow.
- Your main concern is deep jowling rather than loose skin; a deeper lifting approach may suit you better.
- You have an implanted electronic device and no clearance to have RF treatment.
- You need to look photo-ready within a day or two, especially for the eye area.

What are the common side effects of Thermage?
The common ones are temporary redness, mild swelling and some tenderness in the treated area, which usually settle on their own. Most patients return to normal activities the same day.
What are the rare but serious Thermage side effects?
Burns or blisters, and areas that look uneven or hollower, are the outcomes I take most seriously. They are uncommon and are closely tied to how the energy is placed and how well the face was assessed beforehand.
Who is more likely to have Thermage side effects?
People with very thin skin or little facial fat, those with a recent tan or irritated skin, and anyone with implanted electronic devices or metal in the treatment area need extra care or a different plan. That is why I go through a risk check before booking.
Are eye Thermage side effects different from face Thermage?
The eye area uses its own tip and a protective eye shield, and the skin is thinner, so temporary puffiness can be more noticeable. I plan the eye treatment separately from the face for that reason.
What should I do if something feels wrong after Thermage?
