Sagging Face After Weight Loss: Choosing a Treatment When It's Loose Skin, Leftover Fat, or Both — Almira's Case

“I finally lost the weight — so why does my face look looser, not slimmer?” I'm Dr. Hyunji Kim, director of Delphic Clinic in Sinnonhyeon, Gangnam. I trained at Korea University Medical Center, and I consult and treat international patients myself. This comes up constantly now, especially from patients who lost weight quickly over a few months. Almira (@almirainkorea) described both halves of the problem — a lower face that stayed full and looked sagging — and had ONDA 40kJ, Titanium 40kJ and a facial contour injection. Here I explain how I choose a sagging face after weight loss treatment: telling loose skin from leftover fat, which tool suits each, when to wait, and what it costs.

In her own words
My lower face has always been my biggest concern. Even after losing weight, I still had a fuller,sagging-looking lower face.
Loose skin, leftover fat, or both? The first thing I check
Choosing a sagging face after weight loss treatment starts with one distinction. Is the lower face heavy because fat stayed behind, or because skin that once covered more volume has loosened? In Almira's words it was both: “fuller” and “sagging-looking.”
- Mainly fat: soft and pinchable along the jaw, a double chin when you look down, skin still springs back.
- Mainly skin: the face is slimmer, but the jawline blurs and folds appear near the mouth.
- Both: fullness under the chin with a softer jawline — the most common picture after weight loss.
Why the face can look saggier after weight loss
When volume leaves faster than skin can adapt, the skin is left slightly too large for what's underneath. Fast loss makes this more noticeable. At the same time, the lower face often holds on to small fat pockets longer than the body does.
I covered why the lower face is often the last to change in Almira's earlier case. Aliaa's case is a useful comparison: there, ONDA after weight loss was the main tool.
Which treatment fits which picture
- 1
Mainly leftover fat
ONDA for broader fullness, or a facial fat-dissolving injection for a small, stubborn pocket. Sometimes one is enough.
- 2
Mainly loose skin
A skin-firming treatment such as Titanium. Reducing more fat here can make looseness look worse.
- 3
Both, like Almira
Reduce the fat and firm the skin in the same plan, so the slimmer shape doesn't look slack.
Why I add skin-firming when I reduce fat
This is my most common change to a patient's own plan. People who've lost weight often ask only for fat reduction, because fullness is what they see. But if the skin is already a little loose, taking out more volume alone can leave the jaw softer, not sharper.
ONDA's microwaves tighten the dermis while heating fat, and Titanium's three wavelengths support firmness at the surface. Together they cover both layers Almira named.
If you're weighing the injection part, read about the fat-dissolving side effects reviews understate before you decide.
What the three-part plan costs
These are example first-visit menu prices for the same three treatments, not a statement of what Almira paid. A larger injection volume, higher energy or repeat sessions change the total.
- ONDA Lifting (Face) 40kj (First-visit trial)₩270,000≈ $200
- Titanium 40KJ (First-visit trial)₩340,000≈ $250
- Face Fat-Dissolving Injection 3cc: DelQ (ACCU)/DelFit (Non-Steroid)₩30,000≈ $20
Example menu prices for this plan — not a statement of what Almira paid. VAT excluded · the same price for Korean and international patients · regular prices: ONDA face 40kJ ₩400,000, Titanium 40kJ ₩500,000 · the plan is set at the free consultation.
When should you wait before treating a sagging face?
- You're still losing weight — the face will keep changing, so the plan would too.
- You lost a lot very recently; give your skin a few months to settle first.
- The sagging is advanced and folds are deep; a device may only partly help.
- You're pregnant or breastfeeding, or have a pacemaker or metal implants in the area.
Keeping the result: stable weight and maintenance
Fat cells reduced by ONDA's heat don't come back, but the remaining ones can enlarge if weight climbs again. A stable weight protects the result more than any follow-up.
- 1
First days
Mild swelling or tenderness at the injection site is normal. Wash with lukewarm water and use sunscreen.
- 2
Several weeks to 2–3 months
ONDA's fat reduction and tightening become clearer. Compare photos at the same angle and light.
- 3
Afterwards
Some patients return for maintenance. We decide that together at a follow-up.
Is this for you?
Who this combination genuinely suits
Signs it may be time
- Friends say you look tired since losing weight
- Your jawline looks softer now than before the weight loss
- Fullness under the chin stayed while your body slimmed
- You're unsure whether you need fat reduction, tightening or both
A good fit if
- Your weight is now stable after a noticeable loss
- Your lower face looks both fuller and softer than you'd like
- Your skin still has some spring when pinched
- You want a non-surgical plan during a Seoul trip
Not the right first step if: you're still losing weight, you lost a lot in the last few weeks, your sagging is advanced with deep folds, or you're pregnant, breastfeeding or have a pacemaker or metal implants in the area.

“Tell me how much weight you lost and how quickly. It changes whether I start with fat, skin or both — and whether I'd ask you to wait.”
Questions patients ask me about this
What is the treatment for a sagging face after weight loss?
It depends on whether the problem is loose skin, leftover fat or both. Skin-firming treatments like Titanium address looseness, while ONDA or a fat-dissolving injection address leftover fat; many people need a combination.
Why does my face look older after losing weight?
When volume leaves faster than skin adapts, the skin can look slightly loose, especially after quick weight loss. Leftover fat in the lower face can add to the heavy look.
Can fat reduction make facial sagging worse?
If the skin is already loose, removing more volume alone can make the jaw look softer. That's why firming is often added to the plan.
How soon after weight loss can I treat facial sagging?
It's wiser to wait until your weight has been stable for a while. If you're still losing, the face will keep changing and the plan would need to change too.
How much does this plan cost at Delphic Clinic?
On the example first-visit menu, ONDA face 40kJ is ₩270,000, Titanium 40kJ ₩340,000 and a 3cc facial fat-dissolving injection ₩30,000 — ₩640,000 in total, VAT excluded. International patients pay the same as Korean patients.
