Lip Filler Gone Wrong: Lumps, Uneven Lips, Migration and Overfilling, and When Dissolving Is the Answer

I'm Dr. Hyunji Kim, director of Delphic Clinic in Gangnam, one minute from Sinnonhyeon Station. I inject lips myself, and I also see patients who arrive with lip filler gone wrong after treatment elsewhere.
The message usually arrives with three photos taken in bathroom light and a single line: "Has my lip filler gone wrong?" I get versions of it regularly, often within a week of treatment, and sometimes from people who have quietly lived with uneven or puffy lips for a year. This page sorts the common problems, explains what settles and what does not, and is honest about when dissolving is the right step.
Is this swelling, or has lip filler gone wrong?
Lips swell more than almost any other area, and the swelling is often uneven. A lip that looks lopsided on day two can look balanced by day ten. That is why I ask patients not to judge the result, or ask for more product, until the early swelling has had time to settle.
What I watch for is the direction of change. Swelling eases day by day. A lump that stays the same size, a border that stays blurred or a shape that still looks overfilled after the swelling has gone belongs to a different conversation.

Four lip filler problems I see after the swelling has gone
Lumps and beads
Small firm spots you can feel with your tongue are common early on. When they remain visible, the usual causes are product placed too superficially, too much in one spot, or a gel that was too firm for the lip. Some soften with time; persistent visible lumps are often better dissolved than massaged.
Asymmetry
No two lip halves are identical, and filler can make a natural difference more noticeable if the plan ignored it. Mild asymmetry can often be balanced with a small amount of product on one side. Larger asymmetry from uneven placement may need partial dissolving first.
Migration above the lip line
Migration usually shows as a soft, puffy shelf above the upper lip border, which blurs the crisp line that makes lips look natural. It is linked to repeated top-ups, large volumes and placement that pushes product outside the lip. Adding more filler on top rarely fixes it; dissolving the misplaced product usually does.
The overfilled look
Lips that look heavy, project forward or dominate the face are often the result of gradual top-ups rather than one session. The same drift happens elsewhere in the face, which I describe in a piece on pillow face filler and how faces end up overfilled.
Wait, correct, or dissolve: how I decide
The rule underneath the table is simple: I do not add filler to hide filler. If a problem exists because there is too much product or it sits in the wrong place, more product makes the lip heavier and the problem harder to undo.

What dissolving lip filler involves
Hyaluronic acid filler can be broken down with hyaluronidase, an enzyme injected into the area. Before I dissolve, I examine the lips, ask about allergies and previous reactions, and explain that the response varies. Some lips need only one session; others need a second visit to finish the job.
Dissolving can take some of your natural volume impression with it for a while, so lips may look thinner than expected at first. I prefer to let them settle and reassess in their natural state before discussing whether to add anything back, and when we do, it is usually less product than before.

- What would make you decide to stop, or not treat me today?
- If I have a lump or migration later, do you offer dissolving, and who performs it?

Lips I would not inject, and when I would not dissolve
I will not add filler to lips that already have migration or visible lumps until the existing product has been dealt with. I also do not inject lips with an active cold sore or infection nearby, and I will postpone if a patient wants a large change in one session.
On the other side, I do not dissolve lips just because they look swollen in the first days. Dissolving too early, before the true result is visible, can remove filler that would have looked fine. When the problem is a filler type that is not hyaluronic acid, dissolving with hyaluronidase may not work, and that needs a different conversation.

Are lumps after lip filler normal?
Some firmness and small lumps in the first days are common, because of swelling and the gel settling. Lumps that are still clearly visible or easy to feel once swelling has gone down are worth having a doctor examine.
What is lip filler migration?
It is when filler sits outside the intended border, most often as a puffy shelf above the upper lip line. It is more likely after repeated top-ups, large volumes or very superficial placement.
Can hyaluronic acid lip filler be dissolved?
Yes. Hyaluronic acid filler can be broken down with hyaluronidase, an enzyme injection. A doctor should examine the lips first, discuss allergy history and explain that results vary and a second session is sometimes needed.
How soon can I have lip filler again after dissolving?
