Korean Wrinkle Fillers: Fine Lines Need a Different Filler From Deep Folds, and Some Wrinkles Need No Filler at All

I'm Dr. Hyunji Kim, director of Delphic Clinic in Gangnam, one minute from Sinnonhyeon Station. Lifting, laser and anti-aging are the core of my practice, and much of my filler work is about lines rather than volume.
Korean wrinkle fillers come up in my consultation room several times a week, usually from patients who point at two very different things in the same breath: a fine line on the cheek and a deep fold beside the mouth. They expect one syringe to handle both. It rarely does, and sometimes neither line is a filler problem. This page explains how I tell them apart and what I use for each.
Etched line or deep fold: a pinch test in the mirror
My overview of Korean filler areas and products covers the face as a whole. For wrinkles specifically, start with this quick check. Gently stretch the skin on either side of the line with two fingers and watch what happens.
Thin gels for fine lines: the wrinkle-specific options
Fine lines sit in thin skin close to the surface. A firm gel placed there can look lumpy or catch the light, so I prefer options made for the job. Our menu lists Belotero 1cc as a wrinkle-specific filler, and Rejuviel 1cc in a wrinkle version made by the Rejuran maker.
With fine lines I use small amounts and spread them thinly, then review. If a line is part of generally thin, crepey skin, I often suggest a booster for skin quality alongside or instead of filler.
Structural support for nasolabial and marionette folds
Deeper folds beside the nose and mouth are a different job. Our Juvederm and Rejuviel options list the nasolabial folds and marionette lines among their areas, alongside the cheeks and chin. The point is not to fill the crease itself to the brim; it is to restore support so the crease softens.

Wrinkles a filler can't fix
- Lines that appear only when you smile, frown or raise your brows; filling them at rest does not stop the movement
- Folds caused mainly by sagging, where lifting the tissue does more than adding volume
- Crepey, sun-damaged skin covered in fine crinkles; skin quality treatments suit it better
- Lines in an area that has already been filled several times, where adding more risks a heavy look

Who I would turn away from wrinkle filler this week
- Anyone with active infection, a cold sore or irritated skin in the area
- Patients expecting every line to vanish, which tends to lead to too much product
- Faces with old filler that has not been assessed first
- Travellers with an important event the next day, before swelling has had time to settle

Hyaluronic acid filler can be dissolved with hyaluronidase if needed, which is part of why I start conservatively.

Which Korean wrinkle fillers do you use for fine lines?
For fine, etched lines I lean towards the wrinkle-specific options on our menu, such as Belotero 1cc (wrinkles) and Rejuviel 1cc (wrinkles). Which one, and whether filler suits the line at all, is decided after examination.
Are nasolabial folds treated with the same filler as fine lines?
Usually not. A deep fold often needs support placed differently, and our menu lists Juvederm and Rejuviel for areas including the nasolabial folds and marionette lines. The choice depends on the depth of the fold and the face around it.
Can filler fix wrinkles caused by sagging?
Only partly. If a fold is deep because skin and tissue have dropped, adding filler into the fold can make the face look heavy. In that case I usually discuss lifting first.
How soon can I fly after wrinkle filler?
Filler is listed as same-day return, but swelling and small bruises can appear in the first days. If you can, have it done early in your trip rather than on the last day.
Can wrinkle filler be reversed?
Hyaluronic acid fillers can be dissolved with hyaluronidase, which we offer per area. That is one reason I prefer to start conservatively.
