Under Eye Filler Migration: Why It Happens, the Signs to Look For, and What I Do About It

Patients often describe it the same way: the hollow under the eye was filled, it looked good for a while, and now there is a puffy ridge that was not there before. I hear questions about under eye filler migration regularly, sometimes a year or more after the original treatment. This page explains why it happens under the eyes in particular, how to recognise it, and what can be done.
Why the under-eye area is the hardest place to keep filler still
Filler behaves differently depending on where it sits. In my overview of Korean filler areas, products and amounts, the under-eye region is the one where I use the least product and take the most time. Three things explain why.
- The skin is very thin, so even a small amount of product placed too shallow can be seen.
- The area moves every time you blink, squint and smile, which can push product away from where it was placed.
- Hyaluronic acid attracts water, and the under-eye region already tends to collect fluid, especially overnight.
Add repeated top-ups over the years and the total volume can quietly build up. The result is often not a dramatic change but a slow drift from a refreshed look to a puffy one.
Mirror check: is it under eye filler migration or something else?
Use this as a guide for describing what you see to a doctor, not as a diagnosis. Natural eye bags, fluid retention and migrated filler can look alike, and only an examination tells them apart.
Fixing migrated under-eye filler, step by step

Examination
I look at the area in different lights and positions and ask what product was used and when, if you know.

Decide whether to dissolve
If hyaluronic acid filler has migrated, dissolving it with hyaluronidase is usually the cleanest fix.

Let it settle
Swelling after dissolving takes time to go down, and the true shape of the area only shows afterwards.

Reassess
Some patients need nothing more. Others benefit from a small amount of product or a skin-quality treatment later.

What I avoid is adding more filler to disguise the ridge. More product under the eye usually means more water held and more puffiness. The same principle drives my approach to faces that drift into an overfilled look, which I describe in a piece on pillow face filler.

Gentler options than volume for tired-looking eyes
Many people who ask for under-eye filler are actually bothered by thin, crepey or dark-looking skin, not by a deep hollow. For them, I often suggest a skin booster before any filler. On our menu, Ultracol can be used under the eyes, while Juvelook is not used in that area, and Rejuran focuses on skin quality rather than shape.

When a true hollow is the problem and the skin is not too thin, a small amount of filler can still be the right tool. Our menu includes Belotero listed specifically for wrinkles and the under-eye area, and I decide the amount only after examining you.
When I would advise against under-eye filler entirely
I do not recommend under-eye filler for prominent eye bags caused by fat pushing forward, because filler can make the area look heavier. I also hesitate when the skin is very thin, when there is a tendency to morning puffiness, or when a patient already has migrated product that has not been dealt with.
If you have had several rounds of under-eye filler and the area now looks puffy, I would rather dissolve, wait and reassess than add anything. Sometimes the honest outcome is that you look fresher with no filler there at all.

What are the signs of under eye filler migration?
A puffy ridge or sausage-like swelling below the lower lash line, puffiness that looks worse in the morning or after salty food, a bluish tint through thin skin, or a hollow that seems to have moved lower or sideways. A doctor should examine it, because eye bags and swelling can look similar.
Why does under eye filler migrate?
The skin under the eye is thin, the area moves every time you blink and smile, and hyaluronic acid draws in water. Too much product, placement that is too shallow or a firm gel in a thin area all make drifting or puffiness more likely.
Can migrated under eye filler be dissolved?
